TRAILS TRIP APPLICATION

Application must be hand delivered or faxed to the Cleveland Darter Club Office no later than 1:00 p.m. Friday, September 20^th , 2002.

______ Yes, I want to attend the Choo Choo Classic in Chattanooga, Tenessee on November 8-9-10, 2002.

______ Yes, I want to attend the Witch City Open XXIII in Danvers, Massachusetts on November 15-16-17, 2002.

______ Yes, I want to attend the 16^th Greensboro Dart Classic in Greensboro, N. Carolina on November 15-16-17, 2002.

* Departure and Return dates will be confirmed by our travel agent, who will contact you if you are awarded the Trip.  If you elect to take a trip, you will know the exact information approximately one month prior to your trip.  Changes to the Trip itinerary (extra days, etc.) can be made through The Club travel agent, at your expense.  However, you are required to inform the travel agent of all pertinent data (no. of persons and dates or any changes) at the time of the agent’s phone call to you.
